Job description Raya’s Paradise Residential Care Facility is looking for an experienced Full-time Licensed Vocational Nurse Supervisor (LVN) in the Wes Hollywood area. Must be able to perform a variety of resident care duties in a residential care home setting; be able to follow basic instructions and carry out assignments independently; be able to interface appropriately with other staff, residents and family members. General Qualifications: First aid Certification Experience with Eldercare (Dementia) and supervision Current and Active LVN license in the state of California TB clearance & Health screening Finger prints clearance from FBI & DOJ Must be at least 18 years of age. Must be able to walk, bend, kneel, crouch, reach overhead, lift 35 pounds Verification of your legal rights to work in the U.S. (submit I-9 form within 3 working days) Ability to communicate (speak, read & write English). Must possess ability to maintain a good working relationship with associates, residents, residents’ families, and volunteers at all times. One year of experience supervising others, setting performance expectations, collaboration with team, promoting company culture, resolving conflict, etc. Job Specific: Provide care and supervision to residents (Vital sign measure, Injections, skin checks, medication & incontinence management, communication with health care professionals, responsible parties and administrators, diabetes control). Is responsible for the safety and wellbeing of residents (Update Appraisal needs and follows up with facility staff to ensure consistency and accuracy). Assists with medications, centrally storing, distributing and training. Be available for all care plans with families on ongoing basis. Arranges and assists with medical and dental care including appointments. Maintains and supervises house rules for overall protection of residents. Inspects MAR to make sure complete and correct at all times for all residents (order medications, communicate with pharmacy and MD when needed). Follow up & communicates with responsible parties & all MD's orders to make sure in compliance with orders. Must be on call when an emergency call comes in to NURSE’s extension and be available to return and take calls after hours. Assists in continuous observation of residents with regards to their physical/mental/emotional wellbeing. (During hours and after hours as needed). Provides first aid & CPR measures when needed. Notifies administrator of all occurrences with the residents, measure residents’ vital signs daily or as needed, notify MD when needed in case of abnormality. Checks for skin break down, notify MD and administrator. Documents in PCC (Point Click Care) all incidents, assessments and care plans. Check, create and submit all incident and death reports to the Department of Social Services.
